In 1986, something terrible happened aboard Nasa’s Challenger space shuttle as it was launching seven astronauts into space.

It was the space shuttle programme’s first catastrophic loss, and it just so happened on a mission carrying the first private citizen into space, teacher Christa McAuliffe.

What seemed like a freak accident was, in fact, entirely preventable.
